Patient Portal Information

 To use the patient portal, a link to log in will be sent to your email address. Please download the Charm Health app to your mobile device. You may submit a request to change your password at any time by clicking on the option "change/reset password" on the log-in page. 

  • The patient portal facilitates communication between appointments. However, the Portal does not replace your scheduled office appointments. 
  • Patient portal use is limited to non-emergency communication and requests, such as sending messages to your provider, reviewing lab results, requesting prescription refills, and checking appointments.
  • The patient portal is not checked on the weekends or holidays.
  • We expect to respond to communications and requests within 48 to 72 hours.
  • We will not send private health information to your e-mail as this is non-secure. Please log in to the patient portal to review your private healthcare information.
  • We recommend you do not access the patient portal from public computers as there is a potential privacy breach.
